Fatty liver is not a “patent” of fat people

As a clinical nurse in the Department of Hepatology, we are exposed to many patients with liver disease every day. This is not, I have a patient’s family to go to work that day and ask me: “Qin nurse, my father’s liver is not good to go to the hospital, I also did a check by the way, the results found that transaminase is high, B-ultrasound shows moderate fatty liver, I am so Thin, usually eating regularly, no smoking, no drinking, how can you get fatty liver? Is this possible? Fatty liver is not all obese and drinking people will get it?”
Overweight and obesity will get fatty liver, thin people It won’t be. There are not a few people who have this idea. Before many physical examinations, many overweight people usually predict the prophecy: “It is estimated that I am still fatty liver this time.” But the thin people around me will calmly think that they don’t have to worry about it. However, there are indeed some people who are normal or even fat, and who are also fatty liver patients. What is going on?
Mention fatty liver, I believe that everyone is no stranger. In recent years, the incidence of fatty liver has been younger and rising.
Many people think that fatty liver is a patent for obese people. However, this understanding is not comprehensive. It is important to know that although obesity is one of the important causes of fatty liver, lean people can also suffer from fatty liver. Studies have shown that normal-weight fatty liver patients often have problems with insulin resistance and abnormal body fat distribution, and the incidence of hypertension, hyperglycemia, dyslipidemia, and abdominal obesity is not significantly different from that of overweight patients.
What is fatty liver?

Fatty liver refers to a syndrome of hepatic parenchymal cell steatosis and intrahepatic fat deposition caused by various factors. When the fat content exceeds 5% of the liver weight, it is called fatty liver. According to the fat content of the liver, it is divided into light type (containing fat 5% to 10%) fatty liver, medium-sized (containing fat 10% to 25%) fatty liver and heavy (containing fat 25% to 50%) fatty liver. “Lean human fatty liver” refers to a population with a body mass index of less than 24 and having fatty liver. Body mass index (BMI) = body weight (kg) ÷ height (m) squared.
Are all the troubles of the thrifty gene

Have you heard of the “frugal gene”? Studies have shown that our Orientals suffer from fatty liver and are associated with a “thrifty gene” in our body. It is understood that the “frugality gene” is formed in the early days of food shortage, which is beneficial to the human body to store energy for consumption in hunger, but now, when the amount of exercise is reduced and the food is extremely rich, the “frugality gene” continues to play a role. The gene was evolved into a risk factor for fatty liver, type 2 diabetes, and coronary heart disease. The fat tissue in the human body is like a “warehouse” for storing fat, with a certain capacity. If there is too much synthetic fat in the body, which exceeds the storage capacity of the “warehouse”, the fat cells will “run” everywhere. “Run” to the liver, heart, etc., while lean people have less fat tissue, “warehouse” is small, poor ability to store fat, fat is more likely to “run” in the liver, leading to fatty liver.
Be alert to visceral obesity. Although the thin man looks slim, but the body may be a “fat.” The oils in important organs such as their heart, liver and pancreas may be much more than the subcutaneous fat seen by the naked eye. Liver, pancreas, spleen and other visceral tissues in the human body may accumulate oil. Among them, the liver is the main. Because the liver is the hub of metabolism, sugar and fat are digested by the liver. Therefore, the liver is most likely to accumulate oil and cause fatty liver.
Malnutrition is prone to fatty liver. Many people who are thinner are unbelievable after being examined for fatty liver, and even suspect that they have been misdiagnosed. In fact, it is because lean people mostly like vegetarian food, so that nutrient intake can not meet the needs of the body, and it is easy to cause malnutrition fatty liver.
Some people also suffer from chronic intestinal diseases, long-term anorexia, diet, partial eclipse, vegetarian diet, malabsorption syndrome and gastrointestinal bypass surgery, causing hypoproteinemia, lack of choline, amino acids or lipid-producing substances. Form fatty liver. Here to remind everyone that some obese people take “starvation therapy” to lose weight, so that the weight will drop rapidly in the short term, which is very harmful. Because fat moves too fast and exceeds the body’s metabolic capacity, fat will “run” to the liver, leading to fatty liver.
”Fat ectopic” is piled up into fatty liver. Under normal circumstances, the body’s intake of oil is mainly deposited in adipose tissue. However, beyond the limit of adipose tissue absorption, a phenomenon called ‘fat ectopic’ occurs, resulting in fatty toxicity, accumulation of visceral fat, which in turn causes fatty liver, insulin resistance, hyperlipidemia, diabetes and other diseases. .
How to reverse fatty liver

Although fatty liver is seriously threatening our health, don’t worry, because it is treatable, reversible and preventable. Take obesity seriously, especially for visceral obesity, and make progressive and targeted weight loss. Reduces the displacement and shunt of fat from adipose tissue to non-fatty tissue; helps cell metabolism; reduces toxic fatty acids.
Find out the cause and treat the cause. Drinkers should stop drinking alcohol as soon as possible; diabetic patients should control blood sugar and improve insulin resistance; malnourished people should correct malnutrition; obese people should control their weight and gradually lose weight; change the lifestyle of sitting and less moving; change the diet of high fat and high calories Structure; high blood lipids should be rationally used under the guidance of a doctor to reduce blood lipids; avoid contact with chemical poisons and use of hormone drugs.
Develop scientific and reasonable eating habits. A reasonable diet to do: there are thick and thin, not sweet, three or four five, seven or eight full. You can’t give up nutrition for weight loss, and you need to ensure adequate protein intake. Eat less oil, salt, sugar; eat the right amount of meat, fish, eggs, beans and dairy products; eat more fruits and vegetables and coarse grains. Anti-fatty liver foods include oatmeal, corn, kelp, garlic, apple, milk, onion, sweet potato, carrot, hawthorn and so on. These foods help lower blood fat and cholesterol and keep the stool smooth.
Formulate individualized exercise prescriptions, starting from a small amount of exercise, and persevere. It is especially useful for patients with obesity, diabetes, hyperlipidemia, and hypertension. Exercises can use moderate amounts of aerobic exercise. The amount of exercise should not exceed (170-actual age) times/minute, 3 times to 5 times/week, and each time lasts for 20 minutes to 30 minutes.
Eliminate fear and anxiety on fatty liver and maintain a good mentality. The purpose of preventing and treating fatty liver can be achieved through psychological and behavioral interventions to change the bad habits and hobbies of patients with fatty liver and high-risk groups.
Regarding the drug treatment of fatty liver, no special drug treatment is needed for simple fatty liver; patients with liver function damage can choose liver protection and enzyme-lowering drugs under the guidance of clinical professional doctors. Ursodeoxycholic acid, vitamin E, insulin sensitizers, etc. may contribute to the recovery of fatty liver. At the same time, experts suggest: Do not blindly apply traditional Chinese medicine, because some Chinese medicine may damage the liver. After using Chinese medicine, it can also cover up the disease and affect the control of the primary disease.
Fatty liver treatment is critical and prevention is more important. We have summarized the 28-character rule for fatty liver prevention: avoid alcohol, control weight, exercise, reasonable diet, control blood sugar, lower blood lipids, and stay away from poisons.
No matter how fat or thin, you can prevent and reverse fat through lifestyle: keep your mouth open, take your legs, change your life a small step, and make a big step in liver health!
