5 years old, still bedwetting

Pediatric urinary incontinence, also known as enuresis or bedwetting, refers to leakage of urine caused by loss of control of the bladder. At present, the international child urinary control society standard diagnostic criteria for age ≥ 5 years, frequency of enuresis ≥ 1-3 times, month. Nighttime enuresis is moisturized, usually in sleep, also known as nocturnal urinary incontinence; urinary incontinence during daytime waking, also known as daytime urinary incontinence.

Urinary incontinence in children is common, and it is common for children to leak urine during the day. The incidence of 7-year-old girls is 3%, and the incidence of 7-year-old boys is about 2%. Children’s nocturnal enuresis is more common than daytime urine leakage, and the incidence of urinary incontinence in children decreases with age.

Why do children have bedwetting?

The etiology of enuresis in children is still not fully understood. Most of them are related to the delayed development of the urinary control center, and most of the children will recover with age. Its onset is often the result of multiple factors.

The causes of bedwetting in children are classified into functional and organic. The former is related to heredity, which is a symptom of certain diseases. Brain dysfunction caused by congenital brain hypoplasia or acquired factors, or due to the reflex arc of the spinal cord, or due to stimulation of the urinary system and surrounding tissue, may result in bedwetting. Before the age of 3, due to the imperfect development of the central urinary function, reflex urination occurred at this time, it is not the disease. If you still have bedwetting after 5 years old, you should take your child to the hospital for treatment.

What should the child do with bedwetting?

Treatment of urinary incontinence in children includes behavioral therapy, medication, and physical therapy.

●Adjust diet

Limit water intake before going to bed, empty the bladder to reduce the amount of urine stored in the bladder at night. Encourage regular urination during the day and urinate once every 2 to 3 hours to avoid urinary incontinence due to urgency.

● Establish a reasonable life order

Living is normal, avoiding excessive fatigue and nervousness. It is best to stay asleep, so as not to get too familiar at night, it is not easy for adults to wake up and urinate. Do not be too excited before going to bed, put the urination clean before going to bed, and change the wet quilt pants in time.

●Wake up training

Wake-up training is to enable children to form a conditional reflex of “sound stimulation – urinary – awakening – urination”, and to promote the study of children’s ability to control urination at night.

●Medication

Nocturnal urinary incontinence may be treated by increasing the level of vasopressin. Desmopressin is a synthetic antidiuretic hormone with tablets, sprays and nasal drops. It is orally administered 0.2-0.4 mg/time before bedtime and is suitable for nighttime polyuria.

●Physical therapy

Perineal electrical stimulation, biofeedback, neuromuscular regulation techniques and acupuncture help children control their urine.

As the child grows, the bladder’s capacity increases, the body’s natural alarm system activates, the bladder is overactive, the secretion of vasopressin becomes normal, and the signal response to urination time increases, and urinary incontinence gradually disappears. Although most children with urinary incontinence can heal themselves, it is very important to treat children with severe urinary incontinence in a timely manner.
